2.) Select the service Short-Term Case Management
3.) Select Telehealth
4.) Select a date and time that works for your schedule. If you do not find a time that works for your schedule, please contact intake@thehealingprojectmn.org or Scheduling and Intake at 612-778-2754 (call/text).
5.) Enter your contact information and click Request Appointment.
- Our Client Care Coordinator, Alma, can help young adults, college students, and their families apply for food assistance through SNAP. SNAP is a federal program that provides funds to individuals and families in need and helps them purchase food each month. Even if you are not a US Citizen, you may qualify if you are a US citizen or have US-born children.
- If you are willing to sign a release of information with the county that holds financial responsibility for your application, we will help monitor the progress of your SNAP application.
- We recognize food insecurity can lead to symptoms of depression and dementia, such as low mood, fatigue, cognitive decline, and irritability.
- While waiting for your SNAP application to be reviewed and processed by your local county, our agency will provide a gift card to a local grocery store to help offset the cost of groceries. This is a one-time voucher; the amount depends on our current funding and household size.
- We will provide you with a list of local food shelves and potential food delivery options in your area to help offset the cost of groceries while you wait for your SNAP application to be reviewed.
- While meeting 1:1 with you, we will discuss any other concerns you have or resources you might need, such as mental health, housing, child care, hygiene supplies, etc. We will also connect you to community resources.
- Short-term case Management has a maximum time limit of working with clients for 6 months.
